Good post, Steve--I agree that a large part of the problem is that folks who devote their lives to training for marathons (and, if you do it right, it is as demanding as job and family) getting all their self-esteem from being a "marathon", and not wanting that diluted. What I've noticed over the years is that racing, for many, is just the gym, taken outside--whether it's the headphones, cellphone, or the running companions shoulder-to-shoulder, there's no courtesy or recognition that there are other people out there. This results not only in preventing others from passing them and running whatever our race might be (and in a race which is oversubscribed to begin with, it's not as easy as you think to do this when someone is deep in human or cellphone conversation or iPod reverie), but in race courses covered with wet, slippery or (in the Gatorade areas) sticky paper cups, making the whole experience unpleasant because many others have just dropped their half-filled cups once they were done. Then there's the grabbing of extra energy gels on the course, when offered, and snacks at the finish, without regard for the racers yet to pass. My strongest recollection of the NYCMs I've run is not the finish, or the crowds, but my shoes sticking to the ground and (in '04) the unavailability of PowerGel at "the wall" because of the greed of others.
