This post has been really good food for thought. I've thought about it for a few days now, and I've come to the tentative, highly unscientific conclusion that DNSs are pretty good for races. The race gets the entry fee, and in return, has to supply none of the goods. The RD might be nice and offer injured runners a place in the next year's race, but what percentage of folks can/do take advantage of that? And in a race the size of Twin Cities or Grandma's, who notices if a 1,000 people don't show?
