Preparing for Altitude

I've completed nine marathons, the most recent at Richmond in the fall of 1999 (sat out much of 2000 w/ back and plantar fascia problems). I'm training right now with an eye toward the Mile High City in Denver on 9/16. Since I live and train near sea level, in North Carolina, I'm looking for advice from other lowlanders who have run marathons at altitude. What did you do to prepare, what do you wish you had known to expect, what was the altitude's effect on your pace and perceived exertion? (I'm around a 4:00 guy.) Also, anyone who ran the MHCM last year, in its first year, I'd love to hear whether you recommend it -- the comments on the directory page were somewhat mixed.
