Wrong shoe?

The only thing that I could add to the above advice would be to check the appropriateness of your shoe.

I experienced a similar pain to yours, but not nearly as debilitating, once when I ran a marathon in motion control shoes with a hard plastic Champion Chip tied tightly above the shoe laces. That combination was too rigid for my foot's arch. If you are running in a motion control shoe, perhaps your foot is complaining after 12 miles of a too rigid structure. You could then drop down one level to a stability shoe.
