Most marathons anticipate the call of nature. And I've heard of your "situation" before. I don't think it's all that uncommon (do some reading on Grete Waitz (spelling?) in one of her NYC wins when she had a stomach bug). Most of the larger marathons have porta-a-pot stops along the course. If the marathon's well planned and has a detailed course map you may be able to find where the port-a-pots will be before race day. If not, call and ask the organizers if they'll have port-a-pots and where they'll be.

As for your second question, it happens sometimes, but not very often. My number one tip here is move away from the finish line area after you finish as quickly as possible.
