I was fitted for orthotics months ago, so I am used to them & they cause no problems. I also switched to a motion control shoe (I used to wear a cushioned one as I thought I had high arches, but I do tend to roll in slightly). The cause of this was definitely over-training. I did everything wrong in training for NYC2003. I had finished a marathon in March 2003 (my first) and loved it so much I was like Forrest Gump, I just kept running. So I think the reason for the original injury has been remedied. I'm trying to make sure I stretch (I'm not that flexible, and do have tight hams & calves). Guess the best advice is to take it slow. I'm chomping to run in this beautiful fall NY weather, but it is not worth getting hurt again. I, like SMP, am not sure this will ever be 100% again.
