I've run in a marathon (Desert Classic) that only had 300 runners total. It was a partly out and back course, and I was pacing a friend to Boston, so I was never really alone. But if you can't let go of the pain in the body without outside help, then something that small isn't going to work well. At the same time, I've also run Boston. And for me, that was way way to many people. It was too loud, to crowded, just too much. I never really got a chance to go inside myself. It didn't help for me that someone nearby had her name on her shirt, and we had the same name. That got kinda tough.

I've found that the best marathon size for me is about 2000 - 3000 people. It's not that crowded, there's good competition and you have company the whole time. But in the long run, it's a personal choice.
