I haven't seen anyone vomit along a marathon course (though I've seen it at an aid station 2 miles away from the finish of a 50K).
BTW, like Yellowshark, I also use a bit of coffee to move my bowels before a race and have never had a problem. A good side-effect is that a moderate amount of caffine helps the body use fats more efficiently (so I've read) and my long runs go better when I've had some.
What I have seen however:
1) at Chicago 1998, I've seen someone walking in with "muddy" tights (no, the course is all on roads)
2) at Indy in 2000, I saw a runner with her head between her legs. I thought she might be sick, so I looked to see if I could help..then I noticed that she had pulled her tights and underpants down to her ankles! Yep, there was a big "silvery stream" eminating from her...evidently she did not make it to the trees that were only a few yards away from the course! I turned and tried to pretend that I didn't see anything, but I had to chuckle a bit...
