For my first marathon I did most of my training on a treadmill... the biggest thing I noticed is that I felt WAY more beat up running on the road during the race, especially as the road had a serious cant (slant) on the edge...my right ITB was the real loser. What I did to make it just a bit closer to reality was to keep the treadmill at LEAST at a 1% incline to make up for lack of air resistance that you will feel when running, I also changed pace during my longer runs since I knew that there would be crowds, water stops, turns where my pace would change...
I was a single mom at the time and had small children so I just could go out for an hour and leave them alone... now I only use the treadmill when it's dark outside, icy, REALLY cold or storming (lightening)...while it's not the 'best' thing you can do it's better than nothing, it can give you 'hill' workouts if you live in a flat area and it takes away most excuses...the woman who placed third in the US Olympic trials (Magda Lewy-Boulet)did some of her training on the 'mill as she has a young child...
